Official offer form and original cash register receipts must be obtained from participating store. Send in completed offer form and original receipts for a Cubs/Home Run Inn T-shirt. Qualifying purchases must be made between 6/1/09 and 8/31/09, while supplies last. Your completed re-quest must be postmarked by 8/31/09 and received by 9/14/09. Limit two (1) FREE T-shirts per household. If receipt does not specify product names, write the last five (5) digits of the product UPC numbers on the original receipt. To receive your FREE T-shirt, please do the following: 1) Print and complete this official order form, 2) Provide the original cash register receipt(s) dated 6/1/09 - 8/31/09 with required purchases circled, 3) Bring items listed above to: Wrigley Field, Home Run Inn Tent, 1060 W. Addison Street, Chicago, IL 60613 on June 17, 2009, July 2, 2009, July 10, 2009, July 29, 2009, or August 13, 2009. Home Run Inn tent opens 2 hours before each Chicago Cubs game starts and closes 30 minutes after each game begins. Or please do the following: 1) Print and complete this official order form, 2) Send a check or money order for $2.99 and the original cash register receipt(s) dated 6/1/09 - 8/31/09 with required purchases circled, 3) Mail items listed above to: Home Run Inn Cubs T-Shirt Offer, P.O. Box 5125, Clinton, IA 52736-5125. Entries must be postmarked by 8/31/09 and received by 9/14/09.
Purchase five (5) 12 inch Home Run Inn frozen pizzas: Classic, Signature and Ultra Thin varieties are included. Not all participating products may be available at participating retailer.
Return of any t-shirt as undeliverable may result in forfeiture of the offer. No substitutions or transfers allowed. This official offer form is nontransferable. Offer form requests not in compliance with all terms of this offer will not be acknowledged or returned. For mail-in option, allow 4-6 weeks for reward delivery. This offer may not be combined with any other offer. No group or organization requests will be honored. Submissions must be mailed in a first class stamped envelope. Incomplete or illegible requests and invalid addresses as determined by the U.S. Postal Service will not be honored.
